To request Housing Accommodations:

1. Submit a Student Accessibility Accommodation Request Form
2. Log into the Assumption Accommodate System to download the Physician Support Form located in the Resource Library
3. Upload the completed Physician Support form into the Accommodate System
4. Notification of the decision will be shared once the Housing Accommodations Committee has reviewed your application
