To request Dining Accommodations:

1. Submit a Student Accessibility Accommodation Request Form
2. Log into the Assumption Accommodate System to download the Physician Support Form located in the Resource Library
3. Upload the Physician Support form into the Accommodate System
4. A staff member will contact you to discuss next steps
